Welcome
to our practice! Thank you for selecting Piedmont Pain Consultants for
your treatment. To help us fully evaluate your condition, we request
that you bring the following items with you or have the items sent to
our office prior to your appointment.
- Copies of Your Medical Records from Your Referring Physician
Please ask that your referring physician fax or mail your medical records to our office prior to your visit. In many cases, if we do not have these items, your appointment will be rescheduled.
If you saw Dr. Hord at Pain Consultants of Atlanta, you will need to
contact them (404-351-7654) and request that your medical records be
sent to our office.
- X-ray's, MRI's or CT Scans
Please bring your films or CDs with you. Our doctor needs these items to diagnose your condition. If
you don't have them when you come for your appointment, it may prevent
us from beginning appropriate treatment until they can be obtained.
Patients are responsible for their films. Patients must obtain their
films and bring them to our office for their appointment. Remember to
take your films with you when you leave our office. We do not store
films.
- New Patient Packet
Please complete the attached paperwork and bring it with you to your
appointment. If you do not complete the paperwork before your
appointment date, your time at our office may be extended by at least
one (1) hours and may result in you having to reschedule your
appointment.. Also remember to bring your insurance card(s) and picture
identification.

Piedmont Pain Consultants wants to be different from other pain
management clinics in Atlanta. We want to provide you with a caring
environment and the kind of high-quality personal service you should
expect. As we grow, we will try to keep the feel of a small doctor's
office and avoid the pitfalls of those clinics that concentrate only on
the business of medicine. We will be enlisting our patients to help us
identify ways to improve. We are developing web-based feedback forms to
allow you to give us comments and suggestions. In the meantime, we
